The Medical Certificate under sub-section (3) of section 30 of the Act shall be in Form M.C. Con.

The photograph to be affixed to the medical certificate shall be firmly affixed and not merely pinned to the form and the medical practitioner shall affix his signature or seal to the photograph in addition to signing the form.
(3)
The licensing authority may decline to accept a medical certificate of fitness granted more than one month before the date of application for the grant or renewal of a licence, as the case may be.

Any licensing authority may require as a condition of continuing to hold a licence, the holder thereof to furnish a fresh medical certificate in the form set forth in sub-rule (1) above signed as required by sub-section (3) of section 30 of the Act if the licensing authority has reasonable grounds to believe that the holder of the licence is, by virtue of any disease or disability, unfit to act as a conductor of a stage carriage.
